Waikiki Health is seeking a full-time Community Health Worker of the HIV/HEIS Program
Pay rate for this role is $22.00 per hour.
Schedule: Monday Friday, 40 hour work week

The Community Health Worker of the HIV/HEIS Program will work with members of the interdisciplinary team to provide quality care to patients living with HIV/AIDS and affected others to achieve optimal clinical outcomes. This position will assist in providing services during clinic hours, participating in huddles, population-based management, patient intakes, education, HIV rapid testing and counseling, within clinic protocols.
Qualifications
● Transportation and valid drivers license.
● Familiarity with common software programs, (i.e. Windows-based programs) an ability to quickly learn new systems.
● Ability to communicate effectively with patients, community members, and staff members.
● Proficient in computer skills: typing, navigating through the internet and emails.
● OSHA training.
● BCLS biannually.
General Duties include but are not limited to
● The CHW engages in a multi-disciplinary team approach in the development and management of care plans and appropriate medical and psychological interventions and referrals for people living with HIV/AIDS.
● Assess financial eligibility for Ryan White services, assist with enrollment in WH's Ryan White program, and provide education on participant requirements and eligible services.
● Provide HIV, Hepatitis C, Gonorrhea, and Chlamydia counseling and rapid testing in clinical settings in compliance with strict standards of Waikiki Health, the State of Hawaii Department of Health, and the Centers for Disease Control. And perform quality checks of rapid HIV and Hepatitis C test kits, as directed and log daily metrics of rapid test kits in storage.
● Empower patients with education on basic health information, health care coverage options, and access to other health and community resources.
● This position requires the CHW to collaborate with multiple WH providers, other support staff, healthcare data analyst, insurance agencies, partner agencies, and diverse patient populations.
● Other duties as assigned
